EditorApplication.hierarchyWindowChanged is called twice when GameObject is created by right clicking in the hierarchy window
To reproduce:
1. Open attached project
2. Open Custom window
3. Click Create GO, will print once
4. Click Destroy GO, will print once
5. Right click on hierarchy panel and do create empty GO, will print twice
6. Delete it manually (either right-click->delete or cmd+backspace), will print only once

Resolution Note:
The object is created in the Scene then parented to the GO where the right click happens.
The hierarchy actually changes twice.
